Any individual who has completed the application form and agreed to these terms may become a member of the loyalty program and receive a Customer Card.
The application form can be obtained and completed at any Sporta Punkts store. The customer (or their representative) must complete all mandatory fields, indicate the date of completion, and sign the form.

They have read and agreed to these terms.
They consent to the processing of personal data in the scope and manner specified in these terms and in the application form for the purpose of receiving the benefits of the Customer Loyalty Program.
An agreement has been concluded between Sporta Punkts and the customer (or their representative), the content and scope of which are determined by the completed application form and these terms.
They agree to receive commercial communications from Sporta Punkts regarding services and promotional campaigns via the email address and/or phone number provided in the application form. The customer has the right to unsubscribe from commercial communications by sending a request to: info@sportapunkts.lv.
When submitting the completed application form, the customer (or their representative) must present a valid identity document to a Sporta Punkts employee for identification purposes.
Benefits and discounts cannot be combined with other discount programs.

The Customer Card has no expiration date.
The Customer Card may only be used in Sporta Punkts stores.
The Customer Card is not a payment or credit card.
Discounts are not applied when using another person’s Customer Card.
In the event of loss or damage, a new Customer Card may be obtained by completing a new application form.
